Uses and Disclosures of Health Information:
PDR Home Health Services has established policies to guard against unnecessary uses and disclosures of your health information. Our Agency is with compliance to the Privacy Rule of the Administrative Simplification provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996 for purposes of providing your treatment, obtaining payment for your care and conducting health care operations. Upon admission to the Agency the patient will be provided with a copy of the Agency’s Privacy Notice relative to Protected Health Information.
